How do I join the Vital Plan Healthcare Provider program?

To join our Healthcare Provider (HCP) program, complete the online form for Impact, our Affiliate Network provider for HCPs, submit your application, and then email us at [email protected] to let us know. We will respond to you with approvals, send additional information on the HCP program, and set up your unique coupon code.
